Model-based software development
Disease
protocols for chronic conditions can be “modeled” by expert users,
without the need for programming. Variations of protocols for the same
disease are supported, which makes it possible to implement protocols
for e.g. different countries or regulations within the same system.
Support for roles within the care network
The
VitalHealth Platform has been developed with the inherent capability to
support multiple roles, accessible from anywhere. Each participant in
the healthcare chain - whether it is the general practitioner, the
specialist, a registered nurse or a patient - has access to relevant
information via a web-based user interface. The modeling approach
provides the flexibility to adapt the appearance and behavior of the
solutions to each discipline and location in the healthcare chain.
Process-oriented care
Supporting
a healthcare chain requires the ability to facilitate
multi-disciplinary processes with task delegation, irrespective of
whether these are processes take place within an organization (e.g. in
the case of the division tasks between a general practitioner and a
practice assistant) or across organizational boundaries (e.g. when
referrals to the external specialist are needed).
Rule-based decision support
Disease
management requires more than capturing disease specific data such as
Labs and Vitals. To help the medical experts in providing proactive and
high quality care to their patients, the VitalHealth Platform contains
a sophisticated rules engine that can be used to generate healthcare
alerts and recommended actions for e.g. preventive care.
Open platform architecture
A
disease management solution is often part of a larger scope of
information systems. To that effect, the VitalHealth Platform provides
a flexible integration architecture based on industry accepted
standards. This does not only apply to integration with existing
systems, but also to initiatives which have already been started in
order to arrive at national and international standards for the
creation of an infrastructure for the exchange of patient information
and other medical information.
