Vital for Diabetes was released as VitalHealth’s first disease management model. This web based Integrated Care Delivery Network solution provides optimal support to both the medical professionals and the patient through protocol based treatment of the chronic disease Diabetes Mellitus.

The solution enables the creation of a complete and up-to-date electronic diabetes record through quick and efficient structured data input. All disciplines involved in the diabetes care process are supported. The patient can also participate in the care process through a web based patient portal. Vital for Diabetes is based on published guidelines and treatment protocols.
