Individual risk profiles
An important success factor in creating awareness of cardio vascular risk factors is the ability to create individualized risk profiles. These risk profiles can be created and quantified based on well established and accepted guidelines, such as the SCORE method (Systematic Coronary Risk Evaluation, an algorithm for determining cardio vascular risks based on an international research project with a population of over 200,000 subjects). For each individual, both preventive and corrective lifestyle adjustments are suggested, based on the fact that structured coaching and motivation of lifestyle changes have proven to be successful. Next to that, the solution highlights the necessity of medication intervention, such as medication for lowering blood pressure or cholesterol levels.

With Vital for CVRM, patients have access to their own personal health record through a web based patient portal, with the ability to record daily lifestyle and nutrition habits and by submitting result of home measuring devices for weight, blood glucose and blood pressure. Besides that, the portal can offer personal education tailored to a patient's condition. Patient self-management not only motivates the patient, but also provides the medical professionals with a more complete picture of a patient's overall health status.
