Diabetes is a chronic disease that involves a multitude of care disciplines with a significant percentage of routine treatment steps. As part of the national adoption of an integrated diabetes care network model in the Netherlands, VitalHealth developed a solution for physician care groups in the Almelo region that puts the physician in the driver's seat of the overall primary care process. The Arnhem (Netherlands) based rehabilitation center "Groot  Klimmendaal" is one of the pioneers in healthcare IT. Not necessarily surprising, considering that its CEO Guido van der Logt has a strong background in Information Technology. The rehabilitation center - with 120 beds, 90 ad-hoc treatment areas, 4 clinics and 600 employees - provides rehabilitation services to newborn babies, children, adolescents or adults that suffer from one or more severe physical handicaps, caused by e.g. illness, accidents or birth defects. Stichting Huisarten Laboratorium (SHL) - a Dutch organization that offers amongst others laboratory and Information Technology services  to individual physycians and physyican care networks - has been working diligently on enabling the electronic exchange of patient- and patient exam data. The diabetes care networks supported by SHL had an urgent need for the exchange of laboratory results, weight measurements and medication records, for which the systems in place fell short.
