Diagnostic Center Jeroen Bosch Hospital establishes IDN
DCJBZ delivers care innovation together with VitalHealth Software

Diagnostic Center Jeroen Bosch Hospital (DCJBZ) in 's-Hertogenbosch (NL) delivers and implements the Vital for Diabetes integrated care delivery network solution to family practice physicians and nurse practitioners. As part of that offering, DCJBZ facilitates various diagnostic services, such as blood sample collection at one of the 40 collection locations in the region, or taking fundus photos. Besides that, the DCBJZ coordinates the communication between physicians and the Jeroen Bosch Hospital.
Mr. Hans van de Horst, CEO
The initiator of this new IDN solution Mr. Hans van der Horst, CEO of primary care diagnostics at DCJBZ (see picture), is enthusiastic: "We stand 100% behind this initiative; integrated care delivery networks are a main focus in our strategy, and this system is the ideal tool to use. We're expecting many additional family practice physicians to start working with with DCJBZ through the Vital for Diabetes solution.
We offer added value by not only providing typical diagnostics services such as blood sample collections, but also by providing fundus photo services. The photos are stored in the Vital for Diabetes solutions and are directly accessible by the physicians. This functionality is highly appreciated by the physicians."
Lex Bots, physician and Diabetes specialist
The DCJBZ matches its services to the periodic checkup visits handled by the physicians. The center ensures that blood samples are collected on a quarterly schedule, fundus photos are taken annually
and that results are made directly available in the Vital for Diabetes IDN software. "This results in a clear and consistent overview for the physician, physician's assistant and the patient", says Lex Bots, physician and diabetes specialist."

This means that we want to ensure that we optimize the integration between primary and secondary diabetes care. Besides that, the hospital itself is directly interested in making use of the Vital for Diabetes solution for in-patient care.
