Alzheimer Center VUmc
Partnership Vitalhealth and Digital Alzheimer Center

In May of 2010, VitalHealth Software signed an agreement with the Alzheimer in Utrecht (NL), in order to realize the Digital Alzheimer Center. VitalHealth is working together with Ketting Partners in this project.The purpose of the Digital Alzheimer Center is to provide patients secure online access to the Alzheimer Center, enabling them to make a virtual visit before actually visiting the center. The digital center also enables secure communication between patient and providers, including online education concerning their condition, question and answer mechanisms via web consults, and community based forums to communicate with fellow patients. Diagnostic Center Jeroen Bosch Hospital
Dedicated effort to establish integrated care delivery network

The Diagnostic Center Jeroen Bosch Hospital (DCJBZ) in 's-Hertogenbosch (NL) delivers and implements the Vital for Diabetes integrated care delivery network solution to family practice physicians and nurse practitioners. As part of that offering, DCJBZ facilitates various diagnostic services, such as blood sample collection at one of the 40 collection locations in the region, or taking fundus photos. Color your life; online self-help by Mentalshare
Strong in presentation and user-friendliness

"Color your life" is an interactive website developed by the Trimbos Institute, and offered by Mentalshare. Up until the 1st of February 2010, it was subsidized by the Dutch Ministry of Public Health. The objective of the self-help program is to address or reduce episodes of depression for adults, and ultimately to help prevent mental illness. This approach is a good fit with the vision of insurance provider Silver Cross concerning self-help and prevention. Read more.


Assisted Selfcare Institute (sBZ)
E-health application for thrombosis patients and medical professionals

Earlier this month, the Netherlands based stichting Begeleide Zelfzorg (sBZ) (Assisted Selfcare Institute), a national provider of care to thrombosis patients, has launched the Thrombosis Care Circle. The Thrombosis Care Circle supports sBZ's primary and secondary care processes, such as dosage of anticoagulation medication, logistics and administration. The solution facilitates the daily activities of medical professionals, such as pharmacists, family medicine physicians, nurses and medical specialists. Electronic Medical Record for Mental Health facilities

In January of 2010, VitalHealth Software and Accenture started a joint project to develop a new Electronic Medical Record for the mental health sector. The Geriant Instution selected this VitalHealh/Accenture initiative after a stringent selection process. The main drivers for their choice were: the stability and the future proof architecture of the VitalHealth Platform and the proven success of Accenture in developing and implementing software solutions in the healthcare industry and beyond. GGZ Westelijk Noord Brabant
Care delivery network solution for Schizophrenia

In cooperation with the Netherlands based Trimbos Institute, the Julius Centrum and GGZ Breburg Groep, VitalHealth Software has started an innovative project based around the Vital for Schizophrenia solution. Patients are offered several options for self-management, including EMR access, e-consults and online booking of appointments. All disciplines participating in the care process have role based access to the EMR. The main goals are to get a better picture of patient status, more individualized care based on the patient's preferences and reduction of inpatient admissions.



Integrated diabetes care groups
Improved overview and reporting while reducing workload

As a part of a project to adopt a diabetes care network model, VitalHealth Software created software for the physicians in the Almelo region of the Netherlands, that enables them to direct the overal (distributed) treatment process for diabetes patients. After just one year of working with the Vital for Diabetes software, physicians have a much better grip on the diabetes care process, and a better insight in the status of the overall diabetes population. With easy referrals, better reporting to both patient and insurance providers, the software has contributed to a reduced overall workload.



Mayo Clinic, USA
Disease management at point of care

The Mayo Clinic is one of the largest integrated hospitals in the world and works closely together with VitalHealth Software on several projects. Mayo Clinic has a long history in improving and managing the treatment of chronic diseases. Starting 2007, Mayo Clinic implemented VitalHealth solutions for point of care decision support and population management. These solutions are integrated with several clinical applications within Mayo, such as GE Centricity, and have led to improved outcomes, efficiency and patient satisfaction. The Mayo Clinic is a shareholder of VitalHealth Software.



MS Center at VU Medisch Centrum
Transmural patient dossier for Multiple Sclerosis

With the objective of providing an online dossier to Multiple Sclerosis patients and their care givers, the MS Center of the VU Medisch Centrum in Amsterdam turned to VitalHealth Software for the creation of a transmural electronic medical record. Based on the VitalHealth Platform, the web based Vital for Multiple Sceloris solution enables online communication between patients, medical professionals and specialists around a focused, transmural patient dossier.



Onze Lieve Vrouwe Gasthuis
Smoking cessation program for diabetes patients

Both located in the Netherlands, the "Onze Lieve Vrouwe Gasthuis" organization together with the Diabetes Care Foundation Amsterdam have started an initiative for a mobile outpatient clinic for diabetes patients. One of the services offered is a smoking cessation program around VitalHealth's Vital to Quit Smoking solution. The solution assists the coaches and medical professionals with a structured approach to achieve optimal results.



Revalidatie Nederland
EMR for the Rehabilitation sector

VitalHealth Software and its partner Vir e-Care Solutions, together with the "Revalidatie Nederland' organization, have developed an online EMR for the Dutch Rehabilitation sector. All rehabilitation centers in the Netherlands will adopt the new solution. The Rehabilitation EMR offers role based access to patient information, including a patient portal where patients kan access their own medical record, record daily activities and eating habits, or change appointments.

 

Revalidatiecentrum Groot Klimmendaal
A digital, web based patient dossier

Together with VitalHealth, the Netherlands based rehabilitation center "Groot Klimmendaal" developed a digital and web-based Client Historical Status Overview (CHSO). This digitized version of their traditionally paper based client dossier makes optimal use of VitalHealth's powerful dynamic forms functonality, with role based permissions to view, add or change client information. Groot Klimmendaal chose to have this solution hosted by VitalHealth.

 

UMC Utrecht
Online medical record for the dermatology department

The University Medical Center Utrecht in the Netherlands selected VitalHealth for the realization of an online dermatology medical record for care delivery networks. This solution supports all (geographically dispursed) disciplines involved in treating children with atopic eczema and asthma, including direct participation in the treatment process by the patients and their parents.

 

Leidsche Rijn Julius Gezondheidscentra
Self management for chronic disease patients

VitalHealth is working closely together with Netherlands based Leidsche Rijn Julius Gezondheidscentra, the University Medical Center Utrecht, VitaValley, Imtech and Mediportaal, on a project called PAZIO (a Dutch acronym for Patient Oriented Care Information System). One of the more important aspects of PAZIO is an online self-management environment for patients suffering form one or more chronic diseases. The project was nominated for the Dutch "IT-Office Innovation Award 2009".
