SHL and VitalHealth sign cooperation agreement Stichting Huistartsen Laboratorium in Etten-Leur (Netherlands) - delivering support to care givers in primary care - and VitalHealth software – delivering disease management software for health chains – have entered into a cooperation agreement.
The cooperation between SHL and VitalHealth initially focuses on further optimizing SHL’s existing IT infrastructure, to enable further automation and optimization of data collection. Next to that, the physicians will be offered the option to migrate to a more robust solution for integrated care networks (ICN), including protocol based care, task delegation, referrals within the network and patient self-management. Integration with existing systems will be supported, fully retaining historical data collected to date.

New release Vital for Diabetes
The new releases offers a range of reports for caregivers, such as an overview for a family medicine physician of the diabetes patient population, showing which patients also have eye-, foot- or heart problems. The reports list the total number of patients per physician, along with results and averages for e.g. HbA1c and BMI. Vital for Diabetes is now also available in a “standard edition”, particularly suited for individual physicians or physician groups that want to improve diabetes care, but do not participate in an integrated care network. The standard edition is based on the same proven diabetes care protocol, but does not include patient self-management, or referrals inside our outside the care network. Besides functional enhancements this release benefits from several performance enhancements, resulting in an even faster user interface experience.

VitalHealth in Zorgmobiel Amsterdam
VitalHealth solutions are based on well-established and accepted guidelines. The protocol in Vital to Quit Smoking is based on the Minimal Intervention Strategy (MIS), as well as guidelines by Stivoro and the Nederlands Huisartsen Genootschap (Dutch Physician Society). Besides these guidelines, the solution banks on the many years of experience built-up by VitalHealth partner Diagnosis4Health (D4H) with their “quit-smoking” programs. The implementation of Vital to Quit Smoking at Zorgmobiel will be managed by D4H. Zorgmobiel is uniquely flexible with its model of providing care, bringing mobile units close to the patients at various locations in and around Amsterdam, and extending care into the evening hours. This patient oriented approach in combination with the complete care offering including lifestyle coaching is without question unique. Because of its innovative approach, the Zorgmobiel program is financed by insurance provider Agis.

Seminar “Ketenzorg voor de Chronische Patiënt” We'd like to take you back to our June 10 seminar “Ketenzorg voor de chronische patient”. This seminar was organized in cooperation with Diagnosis4Health and held at our corporate headquarters "Landgoed Zonneoord". With more than 100 attendees - amongst which were physicians, care group managers, IT managers and consultants - the seminar was a resounding success! A post-seminar evaluation showed that the attendees found it to be a highly interesting and inspiring event. It was clear that "integrated network care" is a popular concept that attracts a lot of interest, with IT regarded as an important driver to establish a well structured network.

With
the summer and summer vacations just ahead of us, the typical work
schedule often gets a little extra boost to meet deadlines and finish
activities before enjoying time off. At VitalHealth this holds true as
well, evident from the multitude of activities we’ll tell you about in
this newsletter.
SHL
has since long supported family practice physicians with e.g. care for
chronic disease patients. Currently, SHL is involved with 10 regional
diabetes care groups involving about 400 physicians and roughly 30,000
patients. SHL provides diagnostics services (lab work and fundus
photos) as well as several other facilitating services. Amongst these
are the collection of data for outcomes reporting, providing mirror and
benchmark reports and the processing of payments. SHL’s service is
based on the premise that the physicians focus on working with their
Information System, while SHL extracts the required information from
these systems.
On
June 16th 2008, Vital for Diabetes 3.0 was released. This is an
important release that contains many enhancements based on user
feedback, and further improves on the breadth and depth of support
offered to all participants in an integrated care network, such as the
physician, physician’s assistant, dietician, endocrinologist,
optometrist and diabetes nurse.
Zorgmobiel
is an outpatient mobile care unit for diabetic patients, initiated by
the Stichting Diabeteszorg Amsterdam (Diabetes Care Foundation in
Amsterdam) and the ‘Onze Lieve Vrouwe Gasthuis’. Starting June 2nd,
Zorgmobiel offers a program to assist patients in their efforts to give
up smoking, based on the recently released ‘Vital to Quit Smoking’
solution by VitalHealth. Diabetes patients are at a higher risk for
developing cardio-vascular diseases, and smoking increases that risk.
Using the ‘Vital to Quit Smoking’ software, Zorgmobiel hopes to assist
as many people as possible in giving up smoking.
Presenters and presentations
in
Almelo and experienced user of Vital for Diabetes, provided a clear and
objective view on the possibilities of using Integrated Network Care
systems. Marijke van Hees, member of the management team of IZIT in
Twente, presented a view on how to establish a solid IT infrastructure
for regional network care. She explained how the Twente region aims to
establish a regional Electronic Patient Record as well as a Shared
Service Centre for providing healthcare services from a central point.
The picture she painted resulted in constructive discussion amongst the
participants.
