VitalHealth and Aveant - a home care organization based in Utrecht, the Netherlands - recently signed an agreement to start a pilot implementation of VitalHealth's disease management system for diabetes. The involved parties will be Aveant as a home care organization, Aveant's diabetes nurses, family practice physicians and diabetes patients. Amongst others, the project will include the treatment of diabetes according to guideline protocols, decision support, patient self management, outcome measurement reporting and integration with the information systems used by the physicians.
