Online medical record for top quality multidisciplinary care

VitalHealth Software announces the addition of Vital for Eczema to its successful product portfolio. Vital for Eczema has been developed in close cooperation with The University Medical Center Utrecht in the Netherlands. The solution supports all care disciplines involved in providing care for children with severe atopic eczema, including co-morbid conditions such as asthma. It enables the patients (and parents of minors) to actively participate in the care process.

In Vital for Eczema:

• patients have access to their own medical record, including self-management functionality and online communication with medical professionals;
• eleven different care disciplines (including dermatology, child psychology and nurses) can participate in the care process using a single patient focused medical record;
• medical professionals have the ability to conduct online consultations and discussions;
• the referring pediatrician or dermatologist will be kept up-to-date on the progress of treatment;
• workflows are efficiently supported with the help of reminders and alerts.

Vital for Eczema offers rich functionality to both the patient and medical professional. Based on the unique and model driven VitalHealth Platform, the application can easily be tailored to local scenarios.

VitalHealth Software, a joint venture between the Mayo Clinic (USA) and Noaber Foundation (Netherlands) delivers web based software for integrated care delivery networks, disease management and personal health management.
