Development of care network model in cooperation with Trimbos Instituut, Julius Centrum, GGZ Westelijk Noord-Brabant and GGZ Breburg Groep
As part of the Transition Program Long Term Care, VitalHealth is participating in an innovative project called “Schizophrenia Self-Management”. The goal of the project is to establish a transition in the care for and monitoring of independent living patients dealing with schizophrenia.
The first aspect of this transition will be realized by offering these patients several alternative solutions for self-management, such as access to an electronic medical record, online consultation and appointments and online “homework” and questionnaires. A unique aspect of this transition experiment is the fact that caregivers throughout the care network are involved in the establishment of the care model, with direct access to relevant aspect of the electronic records. ‘Schizophrenia Self-Management’ will help caregivers in getting a better picture of this “off-site” population, including optimal support for individual rehabilitation wishes, making every effort to avoid admission of the patients in case of periodical setbacks.
As part of the project, three locations of the participating GGZ institutes in the Netherlands will implement the national multidisciplinary guidelines for schizophrenia care, resulting in evidence based care that enables the use of validated measurements to determine the effects of the care model. This is an example of first class health management, showing that the concept of personal health management is also applicable to patients with chronic psychiatric issues.
VitalHealth Software has accepted the challenge to develop the software model for this project using its future proof modeling platform. The project will be done in cooperation with Netherlands based GGZ Westelijk Noord-Brabant, GGZ Breburg Groep, the Trimbos Instituut and the Julius Centrum for Health Science and Primary Care.
