This month, the implementation of Vital for Diabetes will start at the Leidsche Rijn Julius GezondheidsCentra (LRJG) in Utrecht, the Netherlands. LRJG comprises four health centers in which a total of 20 physicians and 4 nurse assistants will be using Vital for Diabetes.
An important reason to choose Vital for Diabetes was the fact that the physicians in the health centers adopted a new diabetes care protocol. With Vital for Diabetes, the entire diabetes population will benefit from protocol based care.
Patient self-management
A later phase of the implementation will also include the use of patient self-management functionality. With this functionality, the LRJG centers want to provide their patients the possibility to actively participate in their care process. This means that – coordinated with their physician or physician group – they will have suitable means for self-management, combined with protocol based care from the health centers. The patient will be able to log in to a secure portal for on-demand access to diabetes information resources, view parts of their electronic medical record and proactively contribute to the medical record with information recorded via home measuring devices for e.g. blood glucose and weight.
PAZIO - Patient portal initiative
To roll out the self-management functionality to a broad audience, LRJG partners with VitalHealth Software, the University Medical Center Utrecht and Imtech in the PAZIO project. PAZIO is an open patient portal initiative that enables member physicians or physician groups to offer a range of eHealth functionalities to their patients. The VitalHealth patient self-management functionality will be offered by LRJG via PAZIO.
