Starting early May, the primary care physician group of the city of Zeist (Netherlands) will adopt Vital for Diabetes, one of the disease management solutions by VitalHealth Software. The group consists of 33 primary care physicians.

The physician group Zeist already had extensive experience with structured support in diabetes care, by using the Diabetes Care Protocol of Diagnosis4Health (D4H). In cooperation with D4H, VitalHealth has developed automated conversion routines for a smooth transition from the D4H solution to Vital for Diabetes.

The physician group puts a high value on the continuity in the delivery of high quality care. Together with regional partners, like the Diakonessenhuis, agreements have been made about data exchange throughout the network by using Vital for Diabetes. Supported by Diagnosis4Health, the physician group Zeist is highly motivated and engaged in the process of migrating to the Vital for Diabetes Integrated Care Delivery Network solution.
