Online care delivery for MS patients
Based on the VitalHealth Platform
HealthCare Institute Nieuw Unicum in the Netherlands together with patient organization MS-Anders have taken the initiative for a project called MS-Remote, started in November 2009. This project enables medical professionals specialized in care delivery to Multiple Sclerosis (MS) patients to deliver personalized online care to MS patients throughout the Netherlands. It provides patients with a low threshold means to establish electronic contact with their care providers via a secure Internet connection. The project was started with the approval of the Dutch “Zorgkantoor” organization, subsidized for three years by the Dutch “Zorgautoriteit” (HealthCare Authority).
Multiple Scleroris is an auto-immune disease affecting the central nervous system. The disease progresses in unpredictable manners, unique for each patient. MS has a significant impact on many aspects of life. Next to physical manifestations it usually has many psychological and social implications as well. In the MS-Remote project, the Nieuw Unicum, MS-Anders, myCoachconnect and VitalHealth Software organizations are joining forces to delivery high quality personalized remote care, in sync with the patient’s individual treatment plan.
MS patients can visit the www.ms-op-afstand-nl web site at any time to get in touch with a team of specialized nurses and other medical professionals. Relevant patient information is captured in a web based electronic medical record, based on the VitalHealth Platform. This medical record allows individual MS patients as well as their non-professional care providers (such as friends and family) to monitor the status and progression of their disease, manage appointments and evaluate the effectiveness of treatment options or specific activities for managing their disease. In addition, the project will yield a database with knowledge and experience based on feedback from both medical professionals and patients.
Patients interested in participating in this project are invited to register at www.ms-op-afstand.nl. Participation is free of charge, with the assumption that patients will actively contribute to the evaluation of this service. In practice, this means that participants will be contacted occasionally for feedback on the perceived effectiveness of this form of remote care.
