Automated data management and reporting for chronic diseases
VitalHealth and SHL launch a data management portal, with a smooth migration path to a complete care delivery network information system
In cooperation with Stichting Huisartsen Laboratorium (SHL), VitalHealth Software developed a data management portal. This portal can be used by physician groups for automated extraction of data from physician information systems, including automated checking of data integrity and plausibility. The portal also supports reporting functions for quality and process indicators, as well as billing.
The new portal solution offers a unique stepping stone for primary care physician groups towards integrated care delivery networks, with a solid foundation for future enhancements. For instance, it offers a growth path from the data management portal towards Vital for Diabetes, VitalHealth’s information system for integrated diabetes care networks, with complete reuse of data. This way physician groups can migrate to using functionality such as network referrals, while maintaining their current physician information system as a source of data.
Later this year, SHL and VitalHealth will expand the data management portal to include COPD, Cardio Vascular Risk Management and CHF. By linking the data management portal to the respective VitalHealth solutions for chronic disease management, physician groups can adopt the additional functionality when ready.
SHL currently supports about 400 physicians in 12 diabetes care networks with the data management portal. Every 3 months, SHL contacts about 50,000 diabetes patients to come in for labwork and fundus exams. The results of these tests combined with the data gathered from physician visits provide a wealth of data. This data is automatically extracted from the physician information systems and checked for integrity, completeness and plausibility. Missing or implausible data is reported back to the physician via the data management portal, en will be added to the repository after correction. The resulting data store forms the basis for billing and outcomes reporting, which can subsequently be downloaded from the portal.
