Zorgmobiel
is an outpatient mobile care unit for diabetic patients, initiated by
the Stichting Diabeteszorg Amsterdam (Diabetes Care Foundation in
Amsterdam) and the ‘Onze Lieve Vrouwe Gasthuis’. Starting June 2nd,
Zorgmobiel offers a program to assist patients in their efforts to give
up smoking, based on the recently released ‘Vital to Quit Smoking’
solution by VitalHealth. Diabetes patients are at a higher risk for
developing cardio-vascular diseases, and smoking increases that risk.
Using the ‘Vital to Quit Smoking’ software, Zorgmobiel hopes to assist
as many people as possible in giving up smoking.
VitalHealth
solutions are based on well-established and accepted guidelines. The
protocol in Vital to Quit Smoking is based on the Minimal Intervention
Strategy (MIS), as well as guidelines by Stivoro and the Nederlands
Huisartsen Genootschap (Dutch Physician Society). Besides these
guidelines, the solution banks on the many years of experience built-up
by VitalHealth partner Diagnosis4Health (D4H) with their “quit-smoking”
programs.
The implementation of Vital to Quit Smoking at Zorgmobiel will be managed by D4H. Zorgmobiel is uniquely flexible with its model of providing care, bringing mobile units close to the patients at various locations in and around Amsterdam, and extending care into the evening hours. This patient oriented approach in combination with the complete care offering including lifestyle coaching is without question unique. Because of its innovative approach, the Zorgmobiel program is financed by insurance provider Agis.
